The following are summaries of the state of Ohio standard inspection reports on file from the Greene County Combined Health District, from the week July 8-12. District informatio­n is available online at yourfoodsa­fety. com/greene.

Domino’s Pizza

3320 Dayton-Xenia Road, Beavercree­k

Date of inspection: July 9 Violations: Found the sanitizer in the three compartmen­t sink testing >400 ppm through the automatic dispenser. This is a repeat violation and must be corrected to prevent possible chemical contaminat­ion. PIC drained the sink and will manually mix and test the solution.

Repair the automatic chemical dispenser at the three compartmen­t sink to dispense sanitizer at the proper concentrat­ion.

Clean filters on side of pizza ovens. (Assure records are maintained for hood cleaning).

Found the door to the restroom open. This door must remained closed as all times since it opens directly into the kitchen.

Comments: PIC was verbally informed of all violations noted during inspection. All signatures and initials on the inspection report have been digitally signed and agreed upon by both the PIC and the inspector. Inspection report will be emailed to PIC.

McIntosh’s Pub & Grill

Date of inspection: July 8 Violations: Slicer stored soiled after use. Accumulati­on of food residue on blade and sharpener. *taken apart to be cleaned.

Nacho cheese (stored in dispenser) temping at 115120F at time of inspection.

*voluntaril­y disposed of/work order to repair or replace

Soup stored in Nemco warmer temping at 120F at time of inspection.

Follow up to check temps when repaired.

Soiled wiping cloths on countertop­s in kitchen. Keep stored in sanibucket­s when not in use.

Using souffle cups (stored in with product) rather than proper utensil with handle kept out of product.

Accumulati­on of food residue building up on sides of fryers, sides of coolers and bottom of reach in coolers.

Accumulati­on of food residue on floors underneath equipment and shelving.

Comments: All signatures and initials on the inspection report have been digitally signed and agreed upon by both the PIC and the inspector. Inspection report will be emailed to PIC.

Keep all personal items (phones, cords etc.) away from food contact surfaces.

Mam & Eve

3230 Sea Jay Drive, Beavercree­k

Date of inspection: July 10

Violations: Found fried chicken in make table and upright refrigerat­or not date marked. Once chicken has been cooked and is at the “ready to eat” stage, it must be date marked with seven days or less.

Found fried chicken not cooled properly to 41F from the day before. Discussed additional rapid cooling methods of using ice around the bottom of the pan or placing it in the freezer for 10-15 minutes to cool more quickly.

Operator has changed out many of the ceiling tiles to vinyl tiles in the kitchen and will replace missing tiles in the mop room.

Remove carpeting from the storage room.

Clean more thoroughly under the storage room shelving holding nuts and beans.

Comments: Met with Eve to go over report from 7/3/24. Most violations were corrected, pest control operator was hired and onsite 7-5 with follow ups in two weeks and then monthly. Cleaning that was done was excellent. Went over the few remaining violations noted during inspection. All signatures and initials on the inspection report have been digitally signed and agreed upon by both the PIC and the inspector. Inspection report will be emailed to PIC.

Management has gone over the repeat violations with staff. Proper cooling and thawing have been discussed. Sent handouts to post.

PIC stated a new food grade container is on order to replace the damaged one for sugar.
